You have the option to fill out your patient forms ahead of your visit.

Office Policies
 

COVID-19
Employees and patients are asked to wear masks at all times. We take the patient's temperature upon arrival.  We offer telehealth appointments.

APPOINTMENTS
When calling for an appointment, please provide your name, telephone number, chief complaint/reason for visit, as well as any updated contact or insurance information. We ask that new patients arrive 15 minutes before your scheduled appointment time to fill out necessary paperwork.

Please check your insurance card to see if referral is necessary for the initial appointment. Follow up appointments may be required to be scheduled after testing has been completed.


CANCELLATION OF AN APPOINTMENT
If you are unable to attend an appointment, please notify the office at (631) 230-6644 at least 24 hours before your scheduled appointment time.

NO SHOW POLICY
Failure to cancel your appointment with the office of McMonigle Neurology Associates at least 24 hours prior to your scheduled appointment time will result in a “no show” fee of $50. This fee must be paid at the time of your next appointment.

PAYMENTS
McMonigle Neurology Associates accept cash, personal checks, MasterCard, Discover, Visa and American Express. Checks can be made out to McMonigle Neurology Associates.

PRESCRIPTION REFILLS AND PHARMACY INFORMATION
Please inform McMonigle Neurology Associates of which Pharmacy you use and update us if this should change. Please allow one to two business days for refill requests. We encourage our patients to review their medications prior to their office appointments and to request refills at that time, if needed.
